2. OTA Intake Coordinator will contact you to schedule a free 15 minute phone call with the Director who will discuss your concerns and will recommend an evaluation and/or services.
3. Intake Coordinator will schedule appointments based on your availability and OTA’s available openings.
PLEASE NOTE: While we as a provider will verify your insurance information, you, as a member, should call your insurance member services to understand your benefits such as copays, deductibles, visit limits, etc.
